Page 1 is the main page of the survey. It contains the survey instrument and demographic questions, and is customized with the institution or library's logo (provided by you).

Core Questions: The 22-items that form the core of the LibQUAL+(TM) instrument are listed first. If a participant chooses to add local questions to the survey, those local questions will be interspersed with the core items for a total of 27 items in this first section. |

General Satisfaction and Outcomes Questions: This short section includes eight additional survey questions about information literacy and general satisfaction. |

 |
Library Use Questions: These questions ask how often the user patronizes the library (either in person or online) as well as how often they use non-library gateways such as Google(TM) or Yahoo!(TM) for information. |

 |
Demographics Questions: This section is customized for each different institution type (college/university, community college, law, health, sciences, etc.). It includes questions about age, sex, user group (undergraduate, graduate, faculty, etc.), and discipline. The discipline categories may be customized by each participating institution. |

Comments: This open-ended comments box is a key feature of the LibQUAL+(TM) survey instrument. Respondents can use this box to enter their comments about library service and related issues. Survey liaisons have real-time access to these comments, both, while their survey is in progress and after it is completed. |

E-mail address: If a participating institution offers a local incentive prize to respondents, users can include their e-mail address in this space to enter the prize drawing. This item does not appear on the survey if no incentive prize is offered. |
